West Sussex Family Nurse Partnership – FNP

Family Nurse Partnership (FNP) is a home visiting programme for first-time young mums and families.

We are a team of specialist nurses working with first time, young parents here in West Sussex. Please visit Family Nurse Partnership for more information about our work.

We can offer regular home visits to:

  • Help you and your baby to be as healthy as possible.
  • Help you to meet your goals.
  • Help you get support from other services.

We plan our visits around your needs and your baby, starting in early pregnancy and into your baby’s first year of life.

Contact your midwife or health visiting team if you would like to learn more, or email us to find out if FNP is right for you and your baby: SC-TR.fnp@nhs.net
